What Info do I collect?

This policy details only the data I collect from you when you visit or submit information through my website www.consciouslyintegrated.com. My website may collect data from your submission of information through the “Contact” page and web traffic data, which may include but is not limited to collection of your IP address. This policy only applies to data collected through my practice website and this data is not considered Private Health Information (PHI) or covered by Health Information Portability and Accountability Act (HIPAA). Upon enrolling in services, you will be provided with an additional privacy policy that reviews your rights regarding PHI and HIPAA.

How do I process your data?

Data collected by my website either through your submission or monitoring of web traffic may be used in several ways. Information you submit through the “Contact” page will be used to initiate contact with you over phone, email, or text and to coordinate either a consultation or first appointment. You may indicate your preferred method of communication when submitting your inquiry as well as request that I restrict communication to a particular method (e.g. call but do not text). Website traffic data that is collected in aggregate may be used solely by myself for the purposes of developing advertising strategies through advertising to specific zip codes or search engine optimization for the page.

How long do I keep data collected by my website?

Web traffic data, which is not directly identifiable, is kept for a period of two years. Data that you submit through the “Contact” page is kept for a period of at least two years. I may require this data to be submitted again through my Electronic Medical Records (EMR) system, SimplePractice, at which point it becomes considered Private Health Information and is covered by HIPAA.

How can you delete data I collect from you?

Your web traffic data is unable to be deleted due to the fact that it is not personally identifiable. You may request that I delete information that you have submitted through the “Contact” page by submitting a written request via email to ben@consciouslyintegrated.com. You may also request that I delete any correspondence with you via text or email provided that you have not yet enrolled in services. Once enrolled in services, correspondence and contact information may not be deleted by request.

How do I opt out of text messages?

You may request that I stop using text messages to contact you at any time by submitting this request in writing either through email or text.

How do I keep your data safe?

Your data is never shared or sold to third-parties for advertisement purposes. Data submitted through the “Contact” page is stored in a password-protected Gmail account. You play a role in keeping your data safe as well by not submitting any Personal Health Information or other sensitive information through the “Contact” page.

Do I make updates to this notice?

This notice is updated annually and I reserve the right to update the notice at any time with a 6-week notice to clients.
